Becoming an HSCSN Health Provider

Health Services for Children with Special Needs, Inc. (HSCSN) provides care to Medicaid recipients from birth to 26 years of age in the District of Columbia through its network of more than 2,000 providers. We serve more than 5,800 children and young adults with disabilities.

Physicians and dentists who join HSCSN are assigned a provider representative who acts as a point person for providers to improve understanding of our benefits, policies and procedures. Our representatives also conduct on-site office reviews in accordance with District of Columbia rules and regulations.

Credentialing Process

Providers who are not contracted with HSCSN and are interested in joining HSCSN’s provider network, please contact Awa Sall at asall@hscsn.org or 202-495-7570.

  • Licensed Independent Providers must obtain a Council for Affordable Quality Care (CAQH) ID and submit a provider interest form.
  • Primary Care Providers/Physicians who provide well-child care for HSCSN members ages up to 21 years are required by the DC Department of Health Care Finance  to complete the HealthCheck Provider curriculum.
  • Dental Providers must contact the HSCSN Quality Plan Administrators at 202-722-2744.
  • DME Providers: please note that HSCSN's provider network is currently closed for DME providers as of June 23, 2016.

Contracted Providers

Interested parties may apply for participation by completing an application through the Council for Affordable Quality Care (CAQH) at https://proview.caqh.org.

Already a CAQH Provider

Providers who have previously obtained their CAQH ID and are interested in joining HSCSN may submit their CAQH ID via email to tdesta@hscsn.org. Please be sure that CAQH has the most current and accurate information as this will expedite the credentialing process.

Completed applications will be downloaded and processed within 30 days of receipt of the application. You have the right to be advised of your application status and may contact the Credentialing Specialist at tdesta@hscsn.org or 202-974-4693. Once the application process is completed, you will be notified via certified mail or email of the Credentialing Committee’s decision.

Becoming a CAQH Provider

Providers who have not yet obtained a CAQH ID may submit a request via email to tdesta@hscsn.org.

  • Please include the following information on your request:
    • Provider last name and first name
    • Provider address and phone number
    • Specialty
    • Date of birth
    • NPI or SSN
  • You then will be notified of your CAQH ID via email or by phone.
  • Once you have been provided a CAQH ID, you will then need to login to CAQH to complete your provider application. For a quick reference on how to complete your application on CAQH, visit https://proview.caqh.org.
  • Upon completion of your application, CAQH will send you confirmation that they have received your data.
  • Notify the Credentialing Specialist at tdesta@hscsn.org or 202-974-4693 that your application has been completed on CAQH. Please be sure that CAQH has the most current and accurate information as this will expedite the credentialing process.

Credentialing Materials

HealthCheck Provider Education System

The DC Department of Health Care Finance requires all Medicaid Managed Care physicians who provide well-child care for all children and young adults up to 21 years of age who are living in DC and are covered under Medicaid to complete the free, online HealthCheck Provider Education System. You will receive 5 CME credits upon completion. This training must be done once every two years.
